The coverage gap in standard health insurance
Ask most employees what their group health insurance covers and they'll say "hospitalisation." They're essentially correct. A standard group mediclaim policy (GMC) kicks in when an employee is admitted to a hospital for at least 24 hours. It does not pay for a visit to a GP, a diagnostic test, an X-ray, dental treatment, or a course of prescribed medicines — unless these occur as part of a hospitalisation episode.
For many people, especially younger employees, the entire year of healthcare spending happens outside hospital walls. Doctor visits for a fever, pathology tests, physiotherapy for a sports injury, eye check-ups, dental cleaning — none of these are covered by standard group health insurance. Employees bear these costs entirely out of pocket.
What OPD cover provides
OPD (Outpatient Department) cover fills this gap. An OPD benefit provides a defined annual wallet — typically ₹3,000 to ₹20,000 per employee — that can be used for:
- General practitioner and specialist consultations
- Diagnostic tests (blood work, imaging, ECG)
- Prescribed medicines and pharmacy costs
- Physiotherapy and rehabilitation sessions
- Dental check-ups and procedures (if included)
- Optical — frames, lenses, and eye tests
Claims can be made by submitting bills and prescriptions, or — with digital-first providers — by uploading receipts through an app within seconds.
OPD utilisation is dramatically higher than hospitalisation
The average Indian employee visits a doctor 3–5 times per year but is hospitalised once every 7–10 years. This means OPD benefits are used and valued far more frequently than the hospitalisation cover that sits at the heart of most group insurance programmes.
Higher utilisation frequency means employees are reminded of the benefit multiple times a year. This visibility is a powerful driver of benefits satisfaction scores. Companies that offer OPD benefits consistently score higher on employee satisfaction surveys than those offering hospitalisation-only cover with similar total premium spend.
The OPD benefit structure: what to look for
When evaluating OPD benefits as part of your group health programme:
- Annual OPD limit per employee: ₹5,000–₹10,000 is the current market sweet spot for mid-sized companies
- Family inclusion: Is the OPD limit per employee or per family? Per-family limits go further for employees with young children
- Claim process: App-based upload is vastly superior to paper bills and courier; check if digital submission is available
- Teleconsultation: Doctor-on-demand consultation that is free or deeply discounted dramatically increases utilisation and perceived value
- Network discounts: Some OPD programmes include a lab and pharmacy network with 20–50% discounts, stretching the effective wallet further
The combined model
The ideal employee health benefits structure combines:
- Group mediclaim (GMC) for hospitalisation — the financial safety net for major health events
- OPD cover for day-to-day healthcare — the visible, frequently used benefit that drives satisfaction
- Mental health access — therapy sessions, which are OPD in nature but delivered via a specialist platform
These three elements, bundled under a single benefits programme, cover an employee's healthcare journey from annual check-ups through chronic disease management to acute hospitalisation. The total premium cost for a meaningful version of all three is often under ₹15,000–₹20,000 per employee per year — a small fraction of annual salary even for entry-level positions.
Do you need both?
For businesses serious about employee wellbeing, yes. The hospitalisation-only model is a minimum compliance floor, not a genuine care programme. The employees who value their benefits most — and who are most likely to stay because of them — are those who actually use the benefit. OPD benefits create regular touchpoints that make employees feel covered, not just technically insured.
If budget is constrained, start with group mediclaim and a teleconsultation benefit (often bundled at low cost) and upgrade to a fuller OPD wallet at the next renewal as you see utilisation and satisfaction data.
